Helping filling out main page of the Form 25

I am drafting up a new Court Order as both parties have come to an agreement on changes. On the main page of the Form 25 it states,

Pursuant to the Divorce Act(Canada), This Court Orders That:

Pursuant to the Children's Law Reform Act(Canada), This Court Orders That:

Pursuant to the Family Law Act(Canada), This Court Orders That:

I am unsure what to put in those areas.

That would be specific to your case, including what Act you filed your original application under. If you are in Ontario you would be able to talk to Legal Aid for about 20 mins over the phone for free and they should be able to guide you based on the information you provide to them (have your Application and Answer ready, as well as any Endorsements/Orders from the Courts to make the conversation productive.
