Has anyone in the forum filed a motion to have ex supply notice of assessment. Our court order says it has to be done each June to calculate child support. Ex says he pays enough CS as it is and won't supply notice. I have been searching the court forms and other posts in here, but I can't find what I'm looking for. Does anyone know for certain which form I would use. Thank you.
