Joint Divorce- Question about form 8A

Hi there,
If you are doing a joint divorce, does form 8A have to be signed in front of a lawyer or notary public? ....or is form 36 the only form that needs to have the signatures witnessed?
Thanks for your help. Trying to do this ourselves.
