Offer to Settle Form

I wish to make an offer to settle. Could someone please post a template or the form to use to do this (for Ontario). It would be grreatly appreciated.

you can do an offer to settle by way of letter.

Put in the heading "Offer to Settle" and date it, and make sure it is signed by you.

You can state the terms ie: this is an open offer etc.
