Default Appeal

Hi, does anyone know what form to use when appealing to amend a court order made by a Justice in court of queens bench Alberta?
I see 3 forms on their website:
Appeal of Masters judgement or order form23
Appeal of review officers decision form 43
Appeal of assessment officers decision form 46

They don't make it easy, and I only have 5 days, 2 of which gone with weekend.
Thanks

Lots of info.
I guess I don't want to appeal the order, I want to vary/amend it. I've been looking for hours, I do not see a form to apply to vary...
