Default Finalizing divorce after signed settlement order

I am self representing and will like to know how to wrap up the divorce after we just signed our settlement order.

My wife was the applicant on the divorce but our settlement order states that either party can pursue and complete an uncontested divorce after the order has been signed.

I am looking at Form 36, Affidavit for Divorce. Do I list myself as the applicant on this form? Is there also another form I need to fill out. I have not been able to get clear directions on any of the websites I have visited.
