    withdraw the support provisions of the order / agreement / contract dated
    It is the date of the order / agreement / contract. If your order was made April 1, 2015 then that is the date you put. It helps them make sure they are looking at the right order.

    FRO Notice of Withdrawal Question

    Hi All,
    I am in the process of filling out the FRO Notice of Withdrawal form (Form 006E) found here:

    Ontario Central Forms Repository - Form Identification

    I realize this is going to sound dumb, but I want to make sure I fill this out correctly the first time. A quick question for anyone who has done one of these:

    For the 'hereby withdraw the support provisions of the order / agreement / contract date' should I enter the date of the separation agreement order, or the date for the notice I received from the FRO?

    I appreciate any help / advice anyone can provide. Thanks much.
