Someone just asked me for the link which I neglected to provide on my original post.

This post is way old but I had the same problem recently and it took me forever to figure it out. The cover sheet is called Form 4A and the back sheet is called Form 4C. They are NOT under the Family Court Forms. They are actually under the Civil forms. I'm sure you found them but this post is for anybody with the same issue.

I have been attempting to find a Trial record cover sheet and have been having no luck. does anyone have a link to a site where i can get one?
