    I have just started to fill out my forms and wondering how I can type them online or should i just fill them out in pen.
    I also was wondering about question #19 I want to add the Bio dad to this document and the question asks if all parties have been added

    My ex wife only wanted to add me on the court order and not him and I feel like he needs to be doing his part.

    If the bio dad works and makes money doesnt He have an obligation to support his child?

    I have let my lawyer go due to the fact I can pay him and also pay my monthly CS so I have been starting this on my own

      Also, I agree that the bio-dad should pay child support for his child, and so you should include that in paragraph 19 of the form. I'd add something alone the lines of:
      Last Name, First Name - biological father of said child
      Address (if you have it or can get it somehow)

          I do have the name of the father and address I'm having second thoughts here W out a lwayer??

          She has come after me now for braces and Im sure also college when that time comes around so Its only fair that he be mention to this order and also pay support too

          I have no problem paying but only me NO thats what hurts the most

            Stick to your guns, add the bio father, and clearly indicate that at this time you do not have any contact info, and ask the courts to order disclosure of any past contact info from your ex.
            Also send a request for disclosure to her lawyer to show you had asked but she and/or her council failed to reply, and now you need to ask that she be ordered to provide the same.

              R u saying I should not give the address and name of the father when I write up my Schedule form W form 17a ????

              I would think that by me finding him and seeing that he works and rents an APT in toronto that this would help me in my defence and would hope HOPE that the Judge would make a motion that he should help W CS braces college or whatever

              WOW glad Im a mechanic and not a lawyer these froms suck just trying to stay Positive here!!

                I think what FL meant was if you did not have his address etc then you should give his name & state that you have no other info at this time. If you do have the info, then by all means, include it.

                  Exactly as Andrewskim said.
                  If you know it add it, if not and you only have the knowledge that he lives in TO and appears to be employed, include what you know.

                  I would send a registered letter to the ex's lawyer asking for full disclosure of info on the bio dad. This can be tracked and printed as proof that you requested the info and received no reply. So when you file with the courts you include this documentation and ask for full disclosure and ask that the bio dad be included in the claim for CS as he appears to be employed and living well in Toronto. And as such is legally responsible for the maintenance of his children.

                  I do not think the courts will deny either request.

                      You can also find a ton of information online. I found my son's bio dad with a Google search and inside of 10 minutes had his address, phone number, place of business, wife's name and synagogue. This is useful if they have an uncommon name, but not as easy if they are "Bob Smith".

                        So can I write up a letter myself and without having the courts signing it and send it to him by registared mail and then attach that letter to my form 17a schedule that i made some sort of effort trying to obtain Info from the ex(Lawyer) to get him on this document too?
