Form to increase support voluntarily

Hi all:

First of all I would like to say " Happy new year to all of you".

I have a question if any one know.

What form # is used to increase support voluntarily. I searched on internet. I found Form 15, and 15 D. Is this correct one? Finally, my ex gave me disclosure , and I asked him to increase support voluntarily. If some one guide me , I would appreciate. I live in ontario.

15D is correct. If you both consent, and both have an indepenant witness, it will just be filed and rubber stamped. Read the Family Law Rules rule # 15 carefully.
Take the form to the courthouse, show it to the Duty Counsel and you will get clear instructions on what counter to take it to, etc.

Thanks Mess. Appreciate. I missed to mention that I meant to increase Child support not spousal support. I believe you understood as you may have read my previous post sometimes last year.

The number of posts to a user's name does not designate them as a senior member. Did you have a Chatty Cathy doll when you were growing up?

You are with FRO, correct? What a pain to have to amend a CS order every time it changes.
