I think the major difference is the amount of time you physically have the children, which impacts support. As for decision-making, my understanding is that they are essentially the same: any major decisions have to be made with the consent of both parents.
Joint Custody - both parents have decision making ability and will make all important decisions (religion, heath, education) jointly.
Shared Custody - the child(ren) spend at least 40% of the time with each parent... either 50/50 or 60/40. When you have shared custody, you automatically have the ability to make all decisions jointly.
