Any idea what form I need to submit?

I am back in court on the 19th. Its a follow up for the contempt motion. Judge wanted me to file my motion to change material prior to that date. So I do have that almost finished just waiting for a report.

I have learned today that mom went behind my back and had a meeting with our daughters new ABA provider. Changed the goals, and set up a start date all without informing me or getting my input.

She also kept the child from school on each of her parenting days this week. She informed me that she had a doctors note however when I requested a copy she is saying she doesn't have it.

I do plan to ask in my motion to change for custody. The judge said that the judge at the next court date there could be a temporary order on that issue.

Do I need to file anything more then my motion to change material? Do I file a different type of motion for asking the judge at the next motion to make a ruling especially as mom continues to add to the contempt instead of purging it?

What was the penalty for the mother's contempt? Has there been a sentencing?

Mom's penalty was that her access was decreased to 2 non consecutive days a week for access.

Judge talked to me for a bit about custody and that I was to submit my motion to change to ask for custody. Said that we would be back on the 19th. I had to do my motion to change a minimum 7 days before that hearing.

But the filing office said that I wouldn't be in front of the judge for my motion to change that day. That my motion to change would be going on a different day.
