Checklist for Disclosure for Support
This is a checklist our office uses for determining documents needed for support cases. These are basically the documents listed in sub-section 21(1) of the Child Support Guidelines. You may not need all of these documents or you may need more documents than just this - consider this list a starting point.

For the years 2000, 2001 and 2002, the financial statement of the corporation and its subsidiaries |

where the client receives income from employment insurance, social assistance, a pension, workers compensation, disability payments or any other source, the most recent statements of income indicating the total amount of income from the applicable source during the current ear, or if such a statement is not provided, a letter from the appropriate authority stating the required information |
